Office Policies

Our office policies are designed to ensure clear communication and a seamless experience for all patients.

1. Appointment Timing
  • Late Arrivals: If you arrive late, the session will end as originally scheduled to avoid disruptions to other appointments. Sessions delayed due to provider lateness will still receive their full-time.

  • Missed Appointments: Arriving after the scheduled session has ended may result in the visit being classified as a no-show unless we can accommodate you.

2. Cancellations and Rescheduling

Please notify us at least 24 hours in advance for cancellations or rescheduling. Missed appointments or late cancellations without proper notice will result in a $100 fee. This applies to all visits, including initial evaluations, which typically will not be rescheduled after no-shows.

3. Prescription Refills
  • Refills are ideally handled during appointments.

  • For additional requests, provide 48 hours’ notice. Chronic or long-term medications require an office visit before being renewed.

  • For other refills, contact your pharmacy to send us a fax request.

4. Crisis and Emergency Visits

We strive to address emergencies promptly. However, immediate scheduling may require checking for daily cancellations. Missed appointments during crisis situations may delay rescheduling.

5. Insurance Referrals

Some insurance plans require a referral for your visit. It is your responsibility to ensure this is arranged ahead of time to avoid claim denials.

6. Administrative Service Fees

Tasks such as completing forms, letters, or external agency communications outside appointments are billed at $50 per half-hour. To save time, complete forms in advance and notify us during sessions if additional assistance is needed. Routine tasks like prescription refills and brief follow-ups are excluded.

7. Medical Records Requests

To release your medical records to another healthcare provider, a signed authorization form is required. Please provide the recipient's name, address, and phone number. A $50 fee applies for completing medical paperwork.
